Equity and Safe Sleep for Infants (ESSI)
In November 2023, the Illinois Perinatal Quality Collaborative (ILPQC) launched a new statewide neonatal quality improvement project, the Equity and Safe Sleep for Infants (ESSI) Initiative.
Aim 1
By December 2025, greater than 70% of Illinois birthing and children’s hospitals will address disparities and promote neonatal equity by actively participating in the ESSI initiative.
Aim 2
By December 2025, greater than 80% of infants will be discharged from ILPQC hospitals with a completed ESSI bundle.

What is the ESSI Bundle?
The ESSI Bundle consists of three components …
- Awareness: Parents or caregivers report an understanding of a safe sleep environment
- Readiness: Parents or caregivers are prepared for a transition to home, including SDOH screening and resource linkage
- Transfer of Care: ESSI Newborn Care Plan (Awareness and Readiness) is documented in the discharge summary

Safe Sleep Environment Education and Implementation
- ILPQC Safe Sleep Crib Cards – This file has two options, “Ready for ABCs of Safe Sleep” and “Not Quite Ready for my ABCs of Safe Sleep”. With additional guidelines on the backside, these crib cards serve as an example of educational material that can be utilized by any hospital.
- ILPQC Safe Sleep Door Hanger – A door hanger is a sign that hangs from a door’s handle / knob. Download this Safe Sleep Door Hanger, includes a QR Code to watch safe sleep educational videos.
- ILPQC Safe Sleep Stickers – This ZIP file contains 6 different designs to endorse Safe Sleep. Stickers can be placed on your laptop, working station, baby wipes, and more!
