This collaborative QI call will center on improving screening and rescreening for social determinants of health (SDOH) and enhancing the transfer of care process as families transition home from the NICU or Special Care Nursery. We’ll discuss how to identify and address social needs early, connect families to resources, and ensure a smooth, supported discharge.
